Our Founder – Dr. Jesse Jacobs

Dr. Jesse Jacobs grew up in Flower Mound and is a local product of the North Fort Worth area. He has been deeply involved with sports throughout his life, playing Division 1 College Soccer in Georgia.

Driven by a passion: To help people live free.

Dr. Jacobs has instilled his mission into every FreeForm office – TO HELP PEOPLE LIVE FREE. The freedoms he offers his patients include being free from limitations, medications, and free from dependency on the healthcare system. He believes that when you pursue your full health potential and achieve it, you will be unstoppable.

Dr. Jacobs has established five wellness offices since graduating from Parker University in 2012. FreeForm Chiropractic has four locations in Dallas-Fort Worth, with new offices on the horizon. Dr. Jacobs is honored to work with an outstanding leadership team, the company culture they exemplify, and the lives they have changed at the FreeForm offices.

On a personal note…

Dr. Jacobs and his wife, Emily, have four children: Kai, Koa, Keanu, and Kenna. They love traveling with their family and exposing them to different cultures and experiences. Dr. Jacobs's personal mission is to transform healthcare through leadership and action.

Since receiving his education credentials, Dr. Jacobs dedicated his career to furthering himself as one of the best chiropractors in Dallas, Fort Worth. His passion is raising awareness within his city about their most important asset: their health.

In his spare time, Dr. Jacobs enjoys being outdoors hunting, fishing, golfing, and spending time with his beautiful wife and children. His passion is to help other families thrive by living a healthy and proactive lifestyle.
