Our Team

Dr. Jesse Jacobs is a local product of the North Fort Worth area. He has been highly involved with sports his entire life, playing Division 1 College Soccer in Georgia and returned to Dallas Fort Worth where he graduated from The University of North Texas with a Bachelor of Science in Biology degree.

Dr. Keaton Hildebrand was born and raised in Keller, Texas. He attended the University of Arkansas and graduated with a Bachelor of Kinesiology in Exercise Science. He then moved back to Texas to attend Parker University where he obtained his Doctor of Chiropractic. In his free time, you can find him outside working out, playing pickleball, volleyball, and basketball.

Dr. Roman Rogers was born and raised in San Diego, CA. Dr. Rogers obtained his Bachelor's degree in Kinesiology from California State University San Marcos. He then moved to Dallas, Texas to attend Parker University and pursue his passion for movement, health and wellness. Outside of the office you can find Roman on the golf course, flying planes, or moving around at the gym!

Dr. James Fant is a Keller native who graduated from Timber Creek High School. He received his bachelor's degree in Kinesiology from the University of North Texas. He later went on to graduate from Parker University earning a master's in Human Strength and Performance along with his Doctor of Chiropractic. Patricia, his wife, is his high school sweetheart and together they have a son named Declan. He loves all things outdoors along with spending time with family and friends.

Charity joined the Freeform Family in 2017. She has worked in customer service for 13+ years and is great at connecting with each practice member and putting their concerns at ease. Charity enjoys meeting new people and encouraging healing through chiropractic and natural medicine. She grew up in South Arlington, but Fort Worth is now home. Outside of the office Charity loves going on adventures with her Daughter and spending time with her loved ones.

Diandra has been part of the Freeform Team for 2+ years. She graduated from the University of Texas at Arlington with a degree in Public Relations in 2017. She is passionate about health and wellness, and enjoys spending free time with her fiance' and two pets Bear and Cubs!
